What the inspector wrote

On 4/28/2026 Licensing Program Analyst (LPA) Tasha Alexander met with center director Stephanie Weinmann for an UNANNOUNCED Case Management-incident inspection to discuss the self reported unusual incident that occurred on 4/13/26 when a child was injured on the play ground. Today LPA toured the facility's play ground, reviewed records and interviewed staff. On 4/13/26 while playing on the 2 year old side of the playground, a child was running and ran onto the play structure, slipped on a step,fell forward hitting his forehead on another step causing injury to his forehead. Although there was adequate supervision at the time of the incident, children at this age should be encouraged to play safely and not to run onto play structures that are meant for climbing only. Please see the attached 809-d for Type B citation An exit interview was conducted with center director Stephanie Weinmann A notice of site visit was given and must remain posted for 30 days.

    101229 Responsibility for Providing Care and Supervision(a) The licensee shall provide care and supervision as necessary to meet the children's needs.THIS REQUIREMENT WAS NOT MET AS EVIDENCED BY : a tour of the facility, interviews and record reviews which revealed a 2 year old child ran onto a play structure, fell and injured themselves. Staff did not discourage the child from running onto the structure which is meant for climbing only.
