On 4/28/2026 Licensing Program Analyst (LPA) Tasha Alexander met with center director Stephanie Weinmann for an UNANNOUNCED Case Management-incident inspection to discuss the self reported unusual incident that occurred on 4/13/26 when a child was injured on the play ground. Today LPA toured the facility's play ground, reviewed records and interviewed staff. On 4/13/26 while playing on the 2 year old side of the playground, a child was running and ran onto the play structure, slipped on a step,fell forward hitting his forehead on another step causing injury to his forehead. Although there was adequate supervision at the time of the incident, children at this age should be encouraged to play safely and not to run onto play structures that are meant for climbing only.
