On 10/16/2025 Licensing Program Analyst (LPA) Tasha Alexander met with center director Christina Rodriguez-Pena for a Plan of Correction visit to clear any deficiencies cited during previous inspection on 10/6/2025. On this analyst's Annual/Random inspection on 10/6/2025, records were reviewed and it was found that a staff member did not have a current mandated reporter certificate in file. Today a review of records shows the staff member has updated her mandated reporter training and is now up to date. There are no deficiencies today. Deficiencies were cleared. An exit interview was conducted with center director, Christina Rodriguez-Pena.
