What the inspector wrote

On 8/27/2025 at 9:25AM Licensing Program Analyst (LPA) Jaleesa Jackson arrived for an Unannounced Annual/Random inspection, and met with Director Courtney Ludlow. There were 50 preschool aged children, 8 toddlers and 15 fingerprint cleared staff members present during the inspection. This facility has a toddler option between the ages of 18 months to 36 months. The facility is in ratio at the time of inspection. The facility was toured for a health and safety inspection. Hours of operation are Monday through Friday, 7:30AM to 5:30PM. The facility uses an system called Brightwheel for sign in and sign out in each classroom. LPA toured 4 preschool aged classrooms and 1 toddler classroom. There are adequate play and learning materials available. There is heating/air conditioning, ventilation and lighting. Drinking water is available inside and outside of the center. During todays inspection all toxins, medication, and hazardous material are inaccessible to children. The isolation area for sick children is the director's office. The center has smoke detectors, carbon monoxides, working telephones, and fire extinguishers around the entire facility. There are no pools or other bodies of water at the facility during inspection. LPA Jackson observed the outdoor play area. The facility has two play yards. They are all separated by fencing. There are play structures that have tan bark for cushioning to absorb falls and that are anchored for stability. There is a shaded area provided for the children. There are sandboxes that staff screen before children go outside. Continued 809-C LPA reviewed 5 children's files and 16 staff files. All children's files reviewed are complete. Mandated Reporter training was expired in 11 out of 16 staff files reviewed and 5 staff had an incomplete LIC503 (Health Screening Report). LPA verified both opening and closing staff have current CPR/First aid training. Disaster drills are being conducted at least once every 6 months and the last one conducted was on 4/2025. All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. All required documents are posted in a public accessible area. There were 2 deficiencies cited on today's inspection. See 809-D for deficiencies. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 ( PIN 21-21.1- CCP ). Continued 809-C LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information . LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Courtney Ludlow.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This requirement was not met as evidenced by: Based on interviews, the licensee did not comply with the section cited above 1 child was left without supervision on the playground for 2 minutes which poses/posed an immediate health, safety or personal rights risk to persons in care.
