On 03/05/2026 at 1:30PM, Licensing Program Analyst (LPA) Jaleesa Jackson conducted a Case Management - Incident Visit. LPA met with the Facility Representative Monica Moran and explained the nature of the visit. Present on this visit were 8 staff and 37 school aged children. On 03/02/2026, Facility Representative Monica Moran self-reported an Unusual Incident to the Oakland Southeast Regional Child Care Office via phone. The Unusual Incident was about an accidental confidential information breach. LPA conducted a staff interview about the incident. Based on the staff interview and record review, C1's file with their licensing forms was accidentally made available by email. S1 sent out an email that was intended to be blank licensing forms and accidentally sent C1's completed licensing forms to 14 parents. The staff immediately by email informed the affected people to disregard the email and to delete it. The staff then did a follow up call to confirm the emails were deleted. C1's parent was notified of the incident. The facility has conducted an all staff meeting and all administrative staff have been enrolled in additional training's. The facility has implemented a new policy going forward as well. There was 1 deficiency cited on today's visit. See 809-D for deficiency. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given and discussed. An exit interview was conducted.
