What the inspector wrote

On 4/15/2026 at 2:30PM Licensing Program Analyst (LPA) Jaleesa Jackson arrived for an Unannounced Annual/Random Inspection and met with Licensee Carmen Custodio-Nomellini. LPA informed Licensee of the nature of the visit and was granted entry into the home. Present for this inspection was the Licensee and 2 infants. The home was toured with the licensee to conduct a health and safety inspection. Hours of operation for day care are Monday through Friday, 7:30AM to 5:30PM. ON LIMITS: Converted garage, half bathroom (in converted Garage), Bedroom #1, (end of hallway next the kitchen), Backyard, and Side yard. OFF LIMITS: Kitchen, Living room, Dining Area, Master Bedroom/Bathroom, Bedroom #2, Office, and Bathroom inside home The home has heating and ventilation for safety and comfort. There were age appropriate toys that were observed to be safe and in good condition. There is a fully charged fire extinguisher, working smoke and carbon monoxide detector. Licensee stated that there are no firearms and no one who smokes at the home. Licensee states there are no pets in the home. The facility provides snacks to children. All children that bring their own snacks and lunches from home will be properly labeled and stored. Continued on 809-C LPA reviewed 1 children's file. At the start of the visit 1 child's file was not at the facility. Licensee stated that she gave the packet back to the parent. The original paperwork could not be provided and C1's parent came and filled out the paperwork at the facility. Licensee has not been documenting 15 minute sleep logs for the infants in care. Sleep logs were reviewed during the previous years visit. Licensee's Pediatric CPR and First Aid are completed and expires 2/2027. Licensee has not been documenting disaster drill. There were 4 deficiencies cited during today's visit. See 809-D for deficiencies. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Carmen Custodio-Nomellini, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Carmen Custodio-Nomellini.
