What the inspector wrote

Licensing Program Analyst Cherie Acosta and Kayla Merchant conducted an unannounced case management visit. LPAs met with licensee Shelly Goodrich. There was 1 fingerprint cleared assistant present. There was 11 preschool aged children and 1 infant in care during the visit. During the investigation of a complaint the following violations were observed: Licensee places sheets over the play yards when children are sleeping. LPAs observed sheets placed over 5 preschool children and 1 infant. Licensee was reminded that sheets are not be placed over children's sleeping area. Licensee removed the sheets during the visit. During the investigator's visit on 4/1/25, licensee was operating out of ratio with 12 children without an assistant present. See 809-D for deficiencies cited today. Notice of site visit was provided and must be posted for 30 days. Exit interview and report reviewed with Shelly Goodrich.

This requirement was not met as evidenced by: On 4/1/25 licensee had 12 children in care without an assistant which is a potential risk to the health and safety of children in care.
