What the inspector wrote
On 04/20/2026 at 12:45 PM, Licensing Program Analysts (LPAs) Mone Flores and Kareeca "Reeca" Sykes met with the Licensee, Donise Keller, for an
Unannounced Annual/Random Inspection
. Present during the inspection were four (4) infants and four (4) preschoolers in care with two assistants and the Licensee who are all fingerprint cleared. Residing in the home is the licensee and the licensee's pet dog. Licensee’s home was toured for a health and safety inspection. The facility operates 6AM – 6PM, Monday - Friday.
The home is a one-story home that consists of four (4) bedrooms, two (2) bathrooms, kitchen, dining room, living room, family room, dining area, garage, and fully fenced backyard. The entrance to the day care is the front door. The inside and outside of the home were observed to be neat, clean with age-appropriate materials and toys for the children. Toxins, medications, and hazardous materials were observed to be located in inaccessible areas during today’s inspection.
LPAs observed the following precautions: accessible cabinets and drawers in the bathroom the children use and the kitchen, which is off limits, have safety latches and the electric fireplace is blocked off and made inaccessible to children in care.
Licensee stated there are no firearms in the home and one pet (small dog) in the home. LPAs observed a hot tub located to the right of the backyard which is covered, locked and in an off limit area to children in care.
ON LIMITS AREA:
The living room and dining room, which are the main area of the day care, bedroom 4, the bathroom which is located at the end of the hallway to the right, and left gated side of the backyard.
OFF LIMITS AREA:
Bedroom 1 (Master Bedroom) , Bedroom 2, Bedroom 3, master bathroom, kitchen, dining area, family room, right side of the backyard, and the garage.

ISOLATION AREA:
bedroom 4, located to the right of the entrance, away from children in care.
OUTSIDE PLAY AREA: gated left side of the backyard area which children access from the left side gate.
The home has a fully charged 2A10BC fire extinguisher, a working dual smoke/carbon monoxide detector in the hallway, a working telephone, and all required forms are posted. The licensee conducts and documents fire and disaster drills twice a year with the last one conducted on 10/05/2025
.
The licensee's
CPR and First Aid certificate
is current and expires on
5/12/2027.
The Licensee was reminded of the responsibility as a
mandated reporter
and has provided proof of the required training for all people caring for children which was conducted on
11/24/2025 and expires on
11/24/2027
. LPAs reviewed seven children’s files at 02:20 PM and obtained a current facility roster. The licensee is in compliance with the immunization law.
Facility does provide transportation for children, but licensee understands that children cannot be left alone, unattended in parked vehicles.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

as an additional resource. LPAs also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, the Licensee Donise Keller, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days.
