On 06/03/2026 at 1:30PM, Licensing Program Analysts (LPAs) Mone Flores and Kareeca "Reeca" Sykes met with Licensee Sandra Cardona for an Unannounced 3 Year Required Annual Inspection . Present during the inspection were nine (9) preschoolers and two (2) school age children in care with three assistants and the licensee. Residing in the home is the licensee, licensee's spouse, and one of the licensee's assistants who are all fingerprint cleared. Licensee stated her adult niece is residing in the home for a temporary period and LPAs confirmed she is fingerprint cleared. Licensee’s home was toured for a health and safety inspection. The facility operates 6AM – 6PM, Monday - Friday. The home is a two-story home that consists of four (4) bedrooms, two and a half (2 1/2) bathrooms, kitchen, dining room, family room, living room, laundry area, garage, and backyard. The entrance to the day care is the front door. The inside and outside of the home were observed to be neat, clean with age-appropriate materials and toys for the children. Toxins, medications, and hazardous materials were observed to be located in inaccessible areas during today’s inspection. LPAs observed the following precautions: accessible cabinets and drawers in the kitchen have safety latches and the wood burning fireplace that connects the family room to the living room is blocked off and made inaccessible to children in care. Stairs located near the front door are blocked off by a safety gate and made inaccessible to children in care. Licensee stated there are no firearms in the home. LPAs observed two pets (small dog and cat) in the home. LPAs did not observe a body of water in or around home. ON LIMITS AREA: The living room and dining room, which are the main areas of the day care, kitchen, family room, half (1/2) bathroom, and the fenced backyard, not including the shed located on the left side of the yard. Continued on Page 2 Page 2 OFF LIMITS AREA: Bedroom 1 (Master Bedroom) , Bedroom 2, Bedroom 3, Bedroom 4, the master bathroom, bathroom 2, and the garage. The licensee conducts and documents fire and disaster drills twice a year with the last one conducted on 01/14/2026. The licensee's CPR and First Aid certificate is current and expires on 07/14/2027. The Licensee was reminded of the responsibility as a mandated reporter and has provided proof of the required training for all people caring for children which was conducted on 02/05/2025 . LPAs reviewed six children’s files at 2:20 PM and obtained a current facility roster. The licensee is in compliance with the immunization law.
