On 7/28/2025 at 9:48am Licensing Program Analyst (LPA) Morgan Pringle met with Director Ingrid Escalante for an Unannounced Case Management visit. An unusual incident was self-reported by the facility on 7/22/2025 regarding a lack of supervision. Present during the visit were fifteen (15) infants and six (6) additional staff members. On 7/21/2025 at 5:18pm a child (C1) was left unattended on the infant playground for two (2) minutes after all the other children, seven (7) in total, and two (2) staff members had returned back inside the classroom. LPA was informed that C1 was found by another staff member and brought back to C1's classroom. C1 was was found playing with toys and unharmed. Director informed LPA that extra supervision training was given to staff and additional protocols have been put in place to ensure that all children are accounted for at all times during transitions. Due to the lack of supervision, the facility is being issued a Type A Violation and a $500.00 civil penalty is also being assessed. LPA Pringle informed Director that this report dated 7/28/2025 document(s) 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Pringle informed the Director to provide a copy of this licensing report dated 7/28/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child's file for verification. Notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in the immediate civil penalty of $100.
