What the inspector wrote

On 5/30/2025 Licensing Program Analyst Morgan Pringle met with facility Director Elif Kalkan for a Case Management visit for an incident that was reported to the Department on 5/6/2025. Present during the inspection were eighty-eight (88) preschool age children, forty-three (43) toddlers and twelve (12) additional staff present during the visit. An incident was reported on 5/6/2025 stating a child was left unattended in a bathroom for multiple minutes. The child was found by another staff member while brining another child into the bathroom. It was stated that the child was not in any distress and the parent was notified. LPA was informed that the staff member was standing at the doorway supervising two children in the bathroom. When one child at the toilet closest to the door was finished, the staff member closed the door and walked away. LPA observed the classroom and noted that the bathroom doors are full doors with a tinted window at the top. There are two toilets with a divider separating the two. LPA observed the toilet farthest away from the door cannot be seen when the door is closed. Due to the lack of supervision, the facility is being issued a Type A violation today and a $1000.00 civil penalty is also being assessed as this is a repeat violation from an incident that happened within the past 12 months. LPA Pringle informed Director that this report dated 5/30/2025 document(s) 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Pringle informed the Director to provide a copy of this licensing report dated 5/30/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in the immediate civil penalty of $100. Exit interview and report was reviewed with Director Elif Kalkan.
