What the inspector wrote

On December 12, 2025, at 8:00am, Licensing Program Analyst (LPA) Randy Miranda met with Center Director Elif Kalkan to deliver the findings from a complaint investigation for the above allegation. LPA toured the facility and obtained a copy of the sign-in sheets and facility roster. Present for the inspection were the center director, assistant director, 3 Pedagogista Coordinators, 31 teachers, and 142 children in care (23 infants, 32 toddler age, and 87 preschool age). Based on information obtained through interviews, record reviews, and observations, support staff were unsuccessful in assisting a child with toileting needs. The child began to experience a behavioral episode and became physically upset, prompting the support staff to step away. The ABA Therapist, who had been observing the incident, then intervened and assisted the child with toileting needs. The preponderance of evidence standard has been met. The allegation that Staff did not assist child with toileting is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Article 06, Section 101223(a)(3) is cited on the attached LIC 9099D. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview conducted and report was reviewed with Center Director Elif Kalkan and Assistant Director Roxana Acevedo.
