What the inspector wrote

On September 11, 2025, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux conducted an unannounced 1 Year Required Annual Inspection. LPA was granted entry by licensee, Anna Sarbaz, LPA provide the LIC126. Present during today's inspection was licensee, and two (2) fingerprint cleared assistants and ten (10) preschool age children in care. Facility is in compliance with required ratios today. Days/hours of operation are Monday - Friday from 8:30 am - 6:00 pm. This home is a single story home with 4 bedroom and 2.5 bathrooms. The home was toured for health and safety, home is neat and clear with heating and ventilation for safety and comfort. ON LIMIT AREAS: Main room, Room 1 (adjacent), Half Bathroom, Side Yard OFF LIMIT AREAS : Kitchen, dining room, family room (across from dining room area), all bedrooms (includes the master bedroom and bathroom), dining room, main bathroom and rear yard (main yard), storage unit in the rear of the home. Insolation area: section in the child care room away from the children in care until the parents arrive. The smoke and carbon monoxide detectors located in the main child are room (tested/functioning). The home is equipped with a working telephone, a first aid kit, and fully charged 3A40BC fire extinguisher (located near the half bathroom(, which meets the standards established by the State Fire Marshal. LPA observed sufficient materials, toys, and play equipment. All detergents, cleaning compounds, medications, and other similar items were inaccessible to children. Furniture and equipment were age appropriate and in good condition. Last fire/disaster drill was completed on 06/2/2025. All required postings were observed posted. Licensee states she does not transport children, nor provide over night care. There are no swimming pool/bodies of water, licensee confirmed there are no pool and/or bodies of water at the home. 809-C The outdoor play area was observed to be fully fenced and free from defects or hazardous conditions. The space includes trees that provide ample shade and protection from sunlight, along with age-appropriate play equipment suitable for children of all ages. At 10:30 AM, LPA requested and reviewed the files of three (3) children files and personnel files. All files contained Parent\342\200\231s Rights, Identification & Emergency Information, and Medical Consent forms. During the review, LPA also checked the health records of the licensee, and children and found that the licensee is in compliance with immunization laws applicable to childcare providers. Unusual Incident/Student Injury Report: Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child\342\200\231s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days. The licensee was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com . Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Per licensee does not administer medication at this time. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the LICENSEE, Anna Sarbaz confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. There are no deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, Anna Sarbaz.
