What the inspector wrote

Licensing Program Analyst (LPA) Cherie Acosta and Christina Watts conducted an unannounced case management inspection. LPAs met with Muazzama Afrin Qurashy. LPAs arrived at the facility to investigate complaints. Shortly after LPAs arrived, at approximately 1:25 pm, LPA observed a teacher aide in the hallway with 6 school aged children. The school age teacher was providing supervision in the infant room during this time which left the 6 school children in care without supervision of a teacher. The attached type A violation is cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing. Facility was cited on 9/15/22 for children not being supervised while walking to the restroom. A civil penalty in the amount of $500.00 is assessed today for failing to correct the violation. Exit interview and report reviewed with Muazzama Afrin Qurashy. Notice of Site Visit was provided and must be posted for 30 day.

This required was not met as evidenced by: children in care were left without supervision of a teacher which poses an immediate risk to the health and safety of children in care,
