What Happened
Type A citation issued. Child sustained broken arm in two places after falling from another child's shoulder. Staff member failed to visually supervise children; incident discovered only after school-age children informed staff.
Observations
- Child (C1) fell off another child's (C5) shoulder
- Child sustained fracture in two places
- Staff member (S1) did not witness incident and had to be informed by school-age children
