What the inspector wrote

On 9/16/2025 at 8:55am Licensing Program Analyst (LPA) Morgan Pringle met with facility Director Raquel Acosta for a case management visit for an incident that occurred on 8/28/2025. Present during LPAs visit were seven (7) infants and three (3) additional staff. Facility operates from 7:00am - 6:00pm, Monday - Friday. This Facility is currently under a change of ownership and is currently Gan Aviv by Kid Time. On 8/28/2025 an infant (C1) was given the wrong bottle by a staff member (S1) during feeding time and the bottle may have contained food that the child is allergic to. S1 also signed another staff members name on the feeding chart stating that C1 had been given the correct bottle, which C1 did not. S1 was immediately removed from the classroom and was terminated from the facility. Remaining staff have been reminded about the proper procedures and were also required to retake a training on the facilities procedures. No deficiencies are being cited during this visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with interim Director Raquel Acosta.
