What the inspector wrote

On 9/19/2025 at 8:45am Licensing Program Analyst (LPA) Morgan Pringle met with Director Humaira “Mona” Ahmed for an Unannounced Annual/Random Inspection . Present during the inspection were thirteen (13) preschool age children and two (2) additional staff. Two (2) classrooms, toddler and preschool, were toured for a health and safety inspection. The facility also holds a license for infants (073408233) and operates 7:30am – 6:00pm, Monday – Friday. LPA observed both classrooms to have ample age-appropriate materials for the children’s learning and play that were clean, properly stored and in proper working order. All toxins and hazardous materials were observed to be in inaccessible areas. LPA observed two (2) bathrooms that were observed to be clean, well maintained, with an ample number of sinks and toilets. The diaper changing station in the toddler room was observed to be properly padded, clean, and free from defects. All diapering materials were properly stored as well. All napping materials were observed to be well maintained, properly stored and labeled. The facility provides sleeping mats, and all bedding is brought from the children’s home. There are multiple properly working smoke detectors, carbon monoxide detectors, fire extinguishers, and first aid kits on site. The facility does not transport children. LPA observed the kitchen to be clean, well maintained, free from defects and all hazards have been made inaccessible to the children in care. All food on site was observed to be properly stored. The facility provides two (2) snacks and lunch. All food that is brought from the children’s home was observed to be properly labeled and stored. All counters were observed to be clean and well maintained. All children bring their own water bottles and have access to clean drinking water in and outside of the classrooms. Page 1 of 4 The outside play yards were observed to be fully fenced, well maintained, with ample age-appropriate materials for the children. All play materials for the children were observed to be clean and in good repair. The play structure was observed to be anchored into the ground with ample cushion for children’s safety. There is one (1) locked storage shed that is inaccessible to the children in care. There is plenty of shade for the children. LPA did not observe any bodies of water in or around the facility. At 11:00am LPA asked Director if all preschool age children (age 2-5) play on the play structure that is labeled “for children ages 3-11.” Director stated that all of the 2-year-old children play in the infant yard when the infants are not present and they do not play on the play structure. At 11:15am LPA observed two (2) 2-year-old children on the play structure. LPA asked staff to confirm the children’s names and ages, cross referenced with the children’s roster and stated that the younger children can play on the structure and the staff supervise and help them when needed. ( see LIC809D ). The facility is operating within its licensed capacity and is in ratio. All staff working at the facility have obtained a criminal record clearance or have transferred their clearances. The fire/disaster drill log is complete with the last drill logged 8/14/2025. A physical census of the children was taken and cross referenced with the electronic sign-in and out log. All children were properly signed in by their parent or authorized representative. LPA obtained a sample of the children’s files, staff files, and the facility files. All files were complete. Deficiencies cited during inspection Facility not following play structure age limit rules Director was reminded that California law requires facilities to report unusual incidents and/or injuries to children in care, to the child's parents, and to the Department within 24 hours by phone. Within seven (7) days from the incident, facilities must submit the Unusual Incident/Injury form (LIC 624) to the Department. Director was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Mandated Reporter Training ("Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting https://mandatedreporterca.com/ . Page 2 of 4 Personnel and facility roster must be properly maintained, and fire/disaster drills must be conducted at least every six (6) months and documented. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov . Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Page 3 of 4 Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
