What the inspector wrote

On 9/24/2025 at 2:30pm Licensing Program Analyst (LPA) Morgan Pringle met with Director Humaira “Mona” Ahmed for an unannounced Plan of Correction Visit . Present during the inspection were fourteen (14) preschool age children and one (1) additional staff member. Two (2) rooms were toured. The facility also holds a license for infants(073408233) and operates 7:30am – 6:00pm, Monday – Friday. On 9/19/2025 the following deficiency was cited: Facility not following play structure age limit rules On 9/19/2025 it was observed that the play structure in the preschool yard is labeled, “for children ages 3-11.” Through record review it was found that there are eight 2-year-old children enrolled. Through observation and interview it was found that the 2-year-olds were using the play structure. Today, Director stated that the 2-year-olds enrolled in the preschool will be using the infant yard when the infants are not present. LPA informed Director that a waiver request would need to be made for preschool children to use the infant yard as the infant yard was not licensed for preschool age children. The plan of correction date for the deficiency of 9/23/2025 has been extended until Monday, 10/6/2025. There are no deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Director Humaira "Mona" Ahmed.
