What the inspector wrote

On 10/6/2025 at 11:42am Licensing Program Analyst (LPA) Morgan Pringle met with Director Humaira “Mona” Ahmed for an unannounced Case Management Visit . The facility owner arrived around 11:55am. Present during the inspection were thirteen (13) preschool age children and two (2) additional staff members. The facility also holds a license for infants (073408233) and operates 7:30am – 6:00pm, Monday – Friday. On 9/19/2025 it was observed that the play structure in the preschool yard is labeled, “for children ages 3-11.” Through record review it was found that were are eight 2-year-old children enrolled. Through observation and interview it was found that the 2-year-olds were using the play structure. The following week it was observed the play structure is also labeled “for residential use only.” LPA found that the manufacturer, Gorilla Playsets, states on their website that they only produce and sell “residential playsets.” Gorilla Playsets defines residential playsets as: “A residential playset is designed and certified for use at homes rather than commercial areas such as parks, schools, or businesses. They are safety tested to withstand the use that would come from being in a backyard rather than somewhere that receives high volume of use all throughout the day.” LPA asked facility owner if they were aware of the manufacturer’s statements. Owner stated the play structure was purchased from Amazon and they were not aware of the manufacturer’s statements. Owner stated that she will have the structure removed as soon as possible. Today, a lock was placed on the gate separating the area with the play structure from the patio area of the play area. Page 1 of 2 Due to the information gathered from the manufacturer, the facility must remove the play structure by 10/17/2025 and the area of the play yard with the play structure must be made off-limits to the children in care until the structure is completely removed. Children in care will use the patio area of the play area until the play structure is removed. 1 Type B deficiency is being issued during todays visit. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Director Humaira "Mona" Ahmed. Page 2 of 2
