What the inspector wrote

Licensing Program Analysts (LPA) Cherie Acosta and Diana Campos conducted an unannounced Case Management Inspection. LPAs met with Director Humaira (Mona) Ahmed. During the course of a complaint investigation LPAs observed an infant sleeping in a play yard with a blanket. LPAs observed a second infant in a crib with a small blanket with a stuffed toy attached. The attached type A violation is cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing. Exit interview and report reviewed with Director Humaira (Mona) Ahmed.

    Infant Care Center Sleeping Equipment. Cribs shall be free from all loose articles and objects, including blankets and pillows. This requirement was not met as evidenced by: LPAs observed C1 sleeping with a blanket and C2 sleeping with a small blanket with a toy attached which poses an immediate risk to the health and safety of children in care.
