What the inspector wrote

On 9/19/2025 at 8:45am Licensing Program Analyst (LPA) Morgan Pringle met with Director Humaira “Mona” Ahmed for an Unannounced Annual/Random Inspection . Present during the inspection were six (6) infants and two (2) additional staff. Two (2) rooms were toured for a health and safety inspection. The facility also holds a license for preschool (073408232) and operates 7:30am – 6:00pm, Monday – Friday. LPA observed both rooms to have age-appropriate materials for the children’s learning and play that were clean, properly stored and in proper working order. All toxins and hazardous materials were observed to be in inaccessible areas. The diaper changing table was observed to be clean and properly padded. All diapering materials were properly stored as well. All napping materials were observed to be well maintained, properly stored and labeled. The facility provides sleeping cots, and cribs. There are multiple properly working smoke detectors, carbon monoxide detectors, fire extinguishers, and first aid kits on site. The facility does not transport children. At 9:22am LPA observed three (3) cribs in the napping room. All three cribs had chipped paint and one crib had tape over the rails that was observed to be lifting. One crib was also observed to be broken on the left side. Staff stated that all three cribs are currently being used by the infants enrolled in care ( see LIC809D ). Page 1 of 5 LPA observed the kitchen to be clean, well maintained, free from defects and all hazards have been made inaccessible to the children in care. All food on site was observed to be properly stored. All formulas are brought from home and milk is provided. The facility provides two (2) snacks and lunch. All food that is brought from the children’s home was observed to be properly labeled and stored. All children bring their own water bottles and have access to clean drinking water in and outside of the classrooms. The outside play yard was observed to be fully fenced, well maintained, with ample age-appropriate materials for the children. All play materials for the children were observed to be clean and in good repair. There is plenty of shade for the children. LPA did not observe any bodies of water in or around the facility. At 9:14am LPA observed a storage shed in the yard that had a broken lock making it accessible to the children in care. The contents of the shed were not materials used for outdoor play. There were three high chairs placed in front of the shed ( see LIC809D ). At 9:33am, through record review it was found that two (2) infants under 1 year old enrolled in care, who are able to roll, were not being placed on their back to sleep ( see LIC809D ). At 4:00pm LPA observed that there were no sinks in the infant room. There is also not a sink within arms reach of the changing table. Director stated the staff wash the infants hands in the preschool bathroom, with a hose in the outside yard or with water bottles. Director stated that the sinks were removed last week for remodeling and will be replaced sometime this weekend. LPA did not observe the hose being used or any water bottles in the classroom ( see LIC809D ). The facility is operating within its licensed capacity and is in ratio. All staff working at the facility have obtained a criminal record clearance or have transferred their clearances. The fire/disaster drill log is complete with the last drill logged 8/14/2025. A physical census of the children was taken and cross referenced with the electronic sign-in and out log. All children were properly signed in by their parent or authorized representative. LPA obtained a sample of the children’s files, staff files, and the facility files. All files were complete. Page 2 of 5 Deficiencies cited during inspection Cribs used by infants in care in disrepair Storage shed not made inaccessible to children in care Infant room operating without sinks for one or more weeks Infants under 1 year old, able to roll, not being placed on their backs to sleep Director was reminded that California law requires facilities to report unusual incidents and/or injuries to children in care, to the child's parents, and to the Department within 24 hours by phone. Within seven (7) days from the incident, facilities must submit the Unusual Incident/Injury form (LIC 624) to the Department. Director was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Mandated Reporter Training ("Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting https://mandatedreporterca.com/ . Personnel and facility roster must be properly maintained, and fire/disaster drills must be conducted at least every six (6) months and documented. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov . Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . Page 3 of 5 Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Also, LPA Pringle informed the Director to provide a copy of this licensing report dated 9/19/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
