What the inspector wrote

The attached type A violation is cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing. Notice of Site Visit was provided and must be posted for 30 days. Exit interview conducted and report reviewed with Alma Quintanilla. This requirement was not met as evidenced by: Children are bullied by other children in care which poses an immediate risk to the health and safety of children in care.

The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: some children do not feel safe while in care due to children fighting, which poses an immediate risk to the health and safety of children in care.

    Health-Related Services. The licensee shall immediately notify the child's authorized representative if the child becomes ill or sustains an injury... In the case of less serious injuries including, but not limited to, minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record and notify the child's authorized representative of the nature of the injury when the child is picked up from the center. This requirement was not met as evidenced by: On at least one occasion an incident was not documented and an incident report was not provided to authorized representative.
