On 03/18/21 at 11:30 AM Licensing Program Analysts (LPAs) Monica Mathur and Michelle Sutton conducted an unannounced Case Management Investigation at Footprints Preschool regarding an Unusual Incident self reported by the facility to CCLD about an injury a child (C-1) sustained during outdoor activity time. LPAs met with Director, Traci Weldon and explained the purpose of today's inspection. Present in the facility were 26 children. LPA interviewed the Director, staff, children, and reviewed records. LPA also conducted a physical walk through and observed children in the play yard. There were 9 children with 2 staff present in the play yard at the time of observation. Director reported that on 02/17/21, C-1 and C-2 were playing in the sand box when C-2 hurt C-1 in the eye. S-1 was standing close by, visually supervising children, witnessed the incident, immediately attended to C-1's needs, informed Director, parent and provided medical attention. No deficiency has been cited for this incident. Children's Roster, Personnel Report LIC500, staff records and other information was gathered. Due to technical issues with laptop, report was generated after LPA left the premises. Director was unavailable later in the day to review report. Hence, exit Interview was conducted with the Director on 03/19/21 over phone, report reviewed and discussed. Report to be mailed to obtain signatures and returned to CCLD no later than end of 03/22/21. A NOTICE OF SITE VISIT WAS ISSUED, AND MUST BE POSTED ON OR ADJACENT TO THE INTERIOR SIDE OF THE MAIN DOOR INTO THE FACILITY FOR 30 CONSECUTIVE DAYS.
