Follow-up on corrections·March 24, 2023

On 03/24/2023 Licensing Program Analyst (LPAs) A. Curry and C. Watts conducted an unannounced visit to follow up on a plan of correction that was not cleared. LPA met with the Site Administrator, Antonio Betts, to explain the purpose of today's inspection. The facility was previously cited for operating over capacity. As part of the plan of correction, the facility was instructed to submit a Change of Capacity application by 03/15/2023. The Change of Capacity application has still not been received by the Oakland office. The Site administrator indicated he mailed the application last week, but was unable to provide a certified mail receipt during today's visit. Civil penalties will be assessed today. Civil penalties will continue to accrue $100 per day until application is received. Exit interview conducted, appeal rights were given, and report was reviewed with site administrator Antonio Betts.
