What the inspector wrote

On November 13, 2025 at 11:30AM, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux conducted an Unannounced Annual/ Random Required Inspection . LPA met with facility representative, Antonio Betts and Maria Velez also present at the time of the inspection were 5 staff & seven children (4 infants and 3 toddler age). LPA provided facility representative the Entrance Checklist (LIC 125). The facility was toured to conduct a Health and Safety inspection. The facility currently operates 7:00 am - 6:00 pm, Monday - Friday. The facility has a preschool component #73408252 Classrooms: All classrooms were inspected for age-appropriate furnishings, equipment, & adequate storage for children\342\200\231s belongings. LPA observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and a fully charged 3A40BC fire extinguisher that is accessible throughout the facility. The center is equipped with a fully stocked first-aid kit and available in the classrooms. Facility does not provide transportation. Food Service Areas: All center provided food items are properly labeled & stored separately from cleaning supplies. Food prep area is clean, adequately equipped, & free from hazardous materials. Menu is posted one week in advance, available for review, & dated. Facility supplies snacks. Solid waste bins equipped with tight fitting lid. Bathrooms: Facility has separate staff and child designated bathrooms. Toilets and faucets are in safe and sanitary operating condition. The children are able to reach the sinks and toilets. Supplies are available. There is a diaper change station that is in arm reach of the sinks. Page 1 of 3 ***Continued on LIC 809C*** Outdoor Play Area: There are no other bodies of water, or free-standing water accessible to children. Director confirmed there are no pools and/or bodies of water. There are age appropriate toys and materials for the children. The playground outside is fenced and all equipment and surfaces are free from hazards. Due to today's weather conditions the yards were partially inspected. Records: At 12:45PM LPA requested and reviewed children and staff files. LPA reviewed 4 children\342\200\231s files and 5 staff files. LPA reviewed the facility roster & personnel report. LPA reviewed and qualified staff. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed. Director was reminded of their responsibility to renew CPR/First Aid and Mandated Reporter certificates every two years. The center is in compliance with sign in and out procedure. The facility uses Bright wheel app. Emergency Drills are recorded and performed at least every six months, last one was 10/29/2025. Per facility representative, there are no firearms on the premises. All required documents are posted in a publicly accessible area. Criminal Record Clearance : Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. MyChildCarePlan.org: Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Page 2 of 3 ***Continued on LIC 809C*** Safe Sleep: LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS): Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Per director the facility is not administering medication at the time of today's visit. Unusual Incident/Student Injury Report: Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child\342\200\231s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days. One deficiencies cited during today's inspection: \302\267 Type A Violation: S1 did not have a fingerprint Clearance prior to be in the presence at the facility. LPA Breaux informed Assistant Director, Maria L. Velez that this report dated (11/13/2025) documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Breaux informed the Director to provide a copy of this licensing report dated 11/13/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Appeal rights were given and reviewed. Exit interview conducted and report was reviewed with the Assistant Director, Maria L.
