Facility-requested visit·May 1, 2026

On May 1, 2026, Licensing Program Analyst (LPA) Indira Loza met with Licensee Sandra Hernandez Lopez for a Licensee Initiated Case Management visit. Present during today's visit were three (3) preschool-aged children, one infant, the Licensee, and her fingerprint cleared spouse. LPA toured the facility for a Health and Safety check. The home is single family house consisting of three bedrooms, one bathroom, living room, dining area, kitchen, fully fenced front yard, a walkway on both sides of the house, fully fenced backyard with two storage sheds, and a garage. The Licensee requested to place the Master bedroom located at the end of the hallway from an "off limit" room into "on-limit" status. LPA inspected the room, and observed it to be free from any hazards that could pose a risk to the health, safety, and personal rights of the children in care. Therefore, the room will placed on "on-limit" status. On Limit Areas - Living room, Dining area, bedroom across from the kitchen, the bedroom at the end of the hallway, bathroom, front yard, walk-way on the left of the house, and backyard. Off Limit Areas - garage, walkway to the of right of the house, the bedroom located across from the bathroom, kitchen, and two storage sheds in the backyard. Exit Interview conducted. Report reviewed with Licensee Sandra Hernandez Lopez. A copy of the report was provided. A Notice of Site Visit provided and must remain posted for 30 days.
