Licensing Program Analyst (LPA) Cherie Acosta conducted an unannounced visit in regards to a self reported incident. LPA met with Regional Program Quality Manager KIm Kotas. The facility reported that during outdoor play time a staff brought three children into the classroom to use the restroom. When transitioning from the classroom back outside one child was left in the classroom without supervision. It was discovered that the child was alone in the classroom by a parent. The facility reported the incident to both Community Care Licensing (CCL) and to the parent of the child that was left without supervision. The attached type A violation is cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing. During the visit is was also determined that S1 did not have the required clearance to work in a child care center. A civil penalty of $100 is assessed today. A civil penalty of $250.00 is assessed today for a repeat violation. Facility was previous cited on 9/14/22 under California Code of Regulation, title 22 101229(a)(1) Notice of Site Visit was provided and must be posted for 30 days. Exit interview and report reviewed with KIm Kotas
