On 01/29/2026 at 9:30 AM, Licensing Program Analyst (LPA) Christina Watts conducted an Case Management Inspection at Hope Childcare Center and Preschool. LPA met with Director, Analyn Patten and explained the purpose of today's inspection. During today's inspection, there were 31 preschool children with 5 staff in three classrooms. Director stated there are 39 preschool children enrolled. All staff supervising and caring for children have Criminal Record Clearance. LPA is returning to follow up on an self reported Unusual Incident Report submitted to licensing on 12/12/2025. LPA observed classrooms, reviewed files, conducted interviews and obtained relevant documents. Per interviews, on 12/11/2025, there were 15 preschool children playing outside with 3 staff when 1 staff had to go inside the facility. Interviews stated that C1 was initially riding a bike and then started playing on the scooter outside. Interviews stated C1 started to follow S1 as C1 is fond of S1. Interviews stated S1 was getting a jacket for another child. Interviews stated S1 was helping another child put their jacket on and zipped up. Interviews stated that multiple staff seen C1 next to S1. Interviews stated when S1 was done helping the child with their jacket and just took a step, it was alleged that C1 went up the second stair of the yellow climbing ladder and fell. Interviews stated the staff did not witness the incident occur. California code of regulations, Title 22 stated staff are to supervise children at all times. This facility is out of compliance with California code of regulations, Title 22. LPA Christina Watts informed Director, Analyn Patten that this report dated 01/28/26 with 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. *CON'T ON PAGE 2* *PAGE 2* Furthermore, LPA Watts informed the Director to provide a copy of this licensing report dated 01/28/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. *SEE LIC 809-D FOR DEFICIENCIES* Exit interview conducted and report was reviewed with the Director, Analyn Patten. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
