Correction check·April 23, 2026

On 04/23/2026 at 5:00 PM, Licensing Program Analyst (LPA) Christina Watts conducted a Plan of Correction (POC) inspection at The Garden Community Preschool. LPA's met with Infant Director Jacquelyn "Jackie" Alvarez and explained the purpose of this inspection. Director Haley Daughterty was not present during today's inspection. During today's inspection, there were 8 preschool children in care with 2 staff in 2 classrooms. Infant Director stated there are 44 preschool children enrolled. LPA are following up on 1 Type A citation for lack of supervision and 1 Type B citation for Reporting Requirements violation that the facility was cited for on 04/09/2026. Owner, Melissa Cady submitted the plan of correction on 04/10/2026 via email. During today's inspection, LPA reviewed multiple children's file to verify facility has provided parents the Acknowledgement of Receipt of Licensing Report (LIC 9224) form to sign. LPA observed the Acknowledgement of Receipt of Licensing Report in the children's file. LPA also reminded licensee to provide a copy of the licensing report dated 04/09/2026 that documents a Type A citation(s) to any newly enrolled parents/guardians for 12 months from the date of cited report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. As of 04/23/2026, the deficiencies has been cleared. A clearance letter has been printed and provided to the Infant Director during today's inspection. During today's inspection, there were no violations observed. Exit interview conducted and report was reviewed with the Infant Director, Jacquelyn "Jackie" Alvarez . A notice of site visit was given and must remain posted for 30 consecutive days.
