Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

101223 Personal Rights (a)The licensee shall ensure that each child is accorded the following personal rights (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement is not met as evidenced by: Per LPAs investigation, indoor/ outdoor areas not being sanitized, disinfected, cleaned throughly daily. Staff is not following proper sanitation best practices. Faciilty maintains a daily staff cleaning checklist but does not keep records after 1 week. Recent outbreak of hand, foot, mouth sickened at least 7 contracted the illness. This poses an immediate risk to health safety of children. 101212 Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event [...]. This requirement is not met as evidenced by: Facility failed to notify Licensing Department about an unusual incident involving recent outbreak of Hand, Foot, Mouth illness where at least 7 children fell ill. This poses a potential risk to halth and safety of children in care.
