What the inspector wrote

A Prelicensing Visit was conducted today by Licensing Program Analyst (LPA), Melissa Domantay. LPA met with Applicant, Lita Reeves. An application was submitted for an School-age license. A health and safety inspection was conducted inside and outside. The infant program will operate in 1 classroom, Monday to Friday from 6:00am-8:00am and 11:00am-6:30pm. The measurements for indoors are as follows: INDOORS: 563.14 square feet = 16 SCHOOL-AGE CHILDREN OUTDOORS: To be determined A fire clearance, with a total capacity of 12 infants, ages birth to 24 months, 24 preschoolers, ages 2 years to first grade entry, and 28 school-age, ages 4.9 years to 13 years from the Contra Costa County Fire Protection Department. School-age program operates in 1 classroom. School-age classroom is equipped with varied age appropriate materials and equipment. LPA observed that the classrooms has sufficient lighting and appears to be clean and in good condition. There is 1 toilets and 1 sink located inside the school-age classroom. There is a separate bathroom for staff on site located in the Main Preschool Program, the bathroom on the left, by the entrance door, with 1 toilet, 1 sink. LPA observed an ample supply of age appropriate activities, equipment, furniture, and are equipped with varied age appropriate materials and equipment. The main play yard is located in the middle of the plaza parking lot, which is still under construction and is pending to be measured by LPA. Per Applicant outdoor play yard will be shared between preschool and school-age programs. Applicant will provide a play yard schedule for each program to ensure Preschool and School-age children do not commingle. Applicant is reminded that fencing should be at least 4 feet in height. Facility will provide breakfast, lunch, and AM/PM snacks for children. There is a kitchen available. The building has fully charged 3A40BC fire extinguishers, working smoke and carbon monoxide detector, and first aid supplies. Facility will utilize both manual and electronic sign in/out. See 809-C for continuance. There is a working telephone at the center. Facility will provide water for children, water is accessible to children inside and outside . The isolation area will be determined during final inspection. Director, Timeesha Mays is a fully qualified Director with current CPR/First Aid and mandated reporter training's completed. Applicant is reminded that Director Timeesha's TB clearance results are still needed. Per Applicant, she will provide Director Timeesha's TB clearance results. If this facility plans to provide Incidental Medical Services \342\200\223 IMS. A Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm . Mandated reporter and appeal rights, civil penalties, unusual incident reporting and fingerprint requirements were discussed today. LPAs reviewed responses and provided technical assistance including postings. Licensee is also being informed of the web address ( www.ccld.ca.gov ) for downloading child care forms, and the director is encouraged to email ChildCareAdvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The director is also reminded that mandated reporter training is required for all staff and is to be renewed every 2 years at www.mandatedreporterca.com . School-age license for a capacity of 28 school-age children is pending until outdoor play yard is fully fenced and set up for school-age children to utilize. There are no deficiencies being cited today. A Notice of Site inspection was provided to Applicant, Lita Reeves and exit interview was conducted.
