What the inspector wrote

On 05/29/2026 at 9:40 AM, Licensing Program Analyst (LPA) Christina Watts conducted an Case Management Inspection regarding an alleged lack of supervision violation at Every Child Bay. LPA met with Coordinator, Sara Swaney and explained the purpose of today's inspection. Director, Irene Russo was not present during today's inspection. During today's inspection, there were 10 preschool children with 4 staff in three activity areas. Coordinator stated there are 13 preschool children enrolled. LPA is following up on an self reported Unusual Incident Report submitted to licensing on 04/30/2026. LPA toured the facility, observed classrooms, conducted interviews and obtained relevant documents. On 04/29/2026, interviews stated children were transiting from activity time to lunch and 1 staff were the main area where lunch is served while another staff was in a different activity area and 1 staff was assisting children with the bathroom. Interviews stated C1 went inside the Atelier room (art exploring room) and the door was closed. Interviews stated C1 was left unsupervised in the art room for about 7-8 minutes. Interviews stated when children were sitting down for lunch and staff were counting children when a staff member noticed a child was missing. Interviews stated staff went to look for child and found the child inside the Atelier room, happily playing. California code of Regulation, Title 22 states children are to be visually supervised by staff at all times. This facility is out of compliance with California Code of Regulations, Title 22. LPA Christina Watts informed Facility Coordinator, Sara Swaney that this report dated 05/29/2026 documents a Type B citation. Type B citation(s) are a potential risk(s) to the health, safety, or personal rights of children in care. *SEE LIC 809-D FOR DEFICIENCIES* *CON'T ON PAGE 2* *PAGE 2* LPA reminded the Coordinator to inform the Director that an Director's Packet is required when there has been a change to the Director at the facility. Exit interview conducted and report was reviewed with the Coordinator, Sara Swaney. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
