On 12/12/2025 at 9:30 AM, Licensing Program Analyst (LPA) Christina Watts conducted an Case Management Inspection at Baby Yale Academy LT2. LPA met with Director, Naseem "Seema" Israr and explained the purpose of today's inspection. During today's inspection, there were 1 school age child with 1 staff in one classroom. Director stated there are 18 school age children enrolled. LPA is returning to complete an self reported Unusual Incident submitted to licensing on 09/09/2025. LPA toured the facility, observed classroom, conducted interviews and obtained relevant documents. LPA has determined per interviews that in July 2025, 1 staff gave 2 school age children a vape as an incentive to clean the school age room. Interviews stated that the 2 school age children cleaned the school age room and that the next day as a reward, staff gave school age children 2 vapes. California code of regulations, Title 22 states that a facility is to ensure a healthy and safe environment for children. This facility is out of compliance with California code of Regulation, Title 22. LPA Christina Watts informed Director, Seema Israr that this report dated 12/12/2025 documents a Type B citation. Type B citation(s) are a potential risk(s) to the health, safety, or personal rights of children in care. *SEE LIC 809-D FOR DEFICIENCIES* Exit interview conducted and report was reviewed with the Director, Seema Israr. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
