What the inspector wrote
On Tuesday, September 24, 2024 Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) M. Domantay met with Applicant Haseena Dawud for the purpose of a pre-licensing inspection. Applicant requests to add a Preschool Program to the existing Infant license with toddler option to serve 22 Preschool children from ages 2 years to first grade entry, 14 infants ages 0 months to 24 months, and 15 toddlers ages 18-36 months. The programs will operate from Monday to Friday 7:30a.m to 5:30p.m. The facility is pending an updated fire clearance to reflect the correct capacity numbers for each program. The facility does not provide food, all food is provided from families, from home.
LPA informed Applicant, Haseena that the facility will be converted to a single child care center license under facility number 073408959, therefore LPA will be closing the current infant facility number 073405634.
LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted.
There are 2 indoor areas for the infant program, 3 areas for the toddler program, and 3 areas for the preschool program. LPA observed a sufficient amount of equipment, toys, tables, chairs, cubbies, and napping mats. Applicant plans to care for 7 crib aged infants. LPA observed 7 sufficient cribs. Facility will not be administering medications to children. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. LPA observed a combination smoke/carbon monoxide detector in the preschool room 6. LPA observed a manual sign-in/sign-out system.
Children who become ill during the day will be isolated in Director's office and use the staff restroom, if necessary.

LPA measured all classrooms/areas for each program. The total classroom space for the infant classroom contains a total of 601.70 square feet, which will accommodate Applicant's request for 14 infant children.
The total classroom space for the toddler classroom contains a total of 498.52 square feet, which will not accommodate Applicant's request for 15 toddler children. The total classroom space for the preschool classroom contains a total of 829.28 square feet, which will accommodate Applicant's request for 22 preschool children. There is 2 toilets and 2 sinks for the preschool children, 1 sink for the toddler children, and 1 toilet and 2 sinks in the infant classrooms, and a separate private restroom for the staff is located in the facility.

OUTDOOR ACTIVITY SPACE:
LPA measured all outdoor areas for each program. The outdoor play area is fenced with a wooden fence that is at least four feet tall. There is one play structure in the preschool play yard. There is no play structure in the infant and toddler play yard present during today's visit. Preschool play yard has sufficient matting for cushioning to cushion a child's fall. The infant and toddler play yards has turf for cushioning to cushion a child's fall. LPA observed a sufficient amount of equipment and toys. There are no bodies of water on the premises. LPA reminded Applicant to ensure to empty all water tables when not in use. There is sufficient shading areas provided by trees in the infant and preschool play yards. LPA notified Applicant shading must be installed in the toddler play yard.
The total outdoor space for the infant classroom contains a total of 549.27 square feet, which will not accommodate Applicant's request for 14 infant children. The total outdoor space for the toddler program contains a total of 880.66 square feet, which will not accommodate Applicant's request for 15 children. The total outdoor space for the preschool program contains a total of 1106.61 square feet, which will not accommodate Applicant's request for 22 children. LPAs notified Applicant will need to submit a request for a waiver for the outdoor play areas for all programs to accommodate the maximum capacity of 7 infant children, 11 toddler children, and 14 preschool children in each designated outdoor play yards at any given time with a set schedule in place.

LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.
Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview conducted and report was reviewed with the Applicant, Haseena Dawud.
The following items are required before a license will be issued:
1. Licensing Program Manager (LPM) final file review.
2. Applicant must submit a waiver request for infant, toddler, and preschool outdoor play yards due to insufficient space with a set rotation schedule in place.
3. Applicant must install door alarm on the entrance door in preschool Room 7. Applicant must provide photos and videos of door alarm to LPA.
4. Applicant must install barrier/gate between infant and toddler outdoor play yard. Applicant must provide photos of completed barrier/gate to LPA.
5. Applicant must install additional shading in the toddler outdoor play yard. Applicant must provide photos of additional shading to LPA.
6. Updated/revised fire clearance LIC850 must be received from fire department.
