Page 1 of 3 Licensing Program Analysts (LPAs) Canela and Sykes arrived unannounced on 09/04/2025 at this Family Child Care Home facility for the purpose of conducting a Case Management - Licensee Initiated inspection. Licensee/Applicant has applied for a Large Family Child Care Home with a capacity of fourteen 14. Upon arrival LPAs were greeted by Licensee Ashley Cohee. Licensee stated there are a total of 7 children currently enrolled at the facility. Present during today\342\200\231s inspection were a total of 5 children, 4 pre-school age, and 1 school-age. Facility hours of operation are Monday \342\200\223 Friday from 6:00 am to 6:00 pm. This is a two story home. The second story of the home is off-limits to day care children. On-limit areas (accessible to children) : 1st Floor- family room, hallway bathroom, kitchen, playroom, and backyard. Off-limit areas (inaccessible to children) : 2nd Floor - Bedrooms, bathrooms, and attached garage on first floor. Isolation Area : couch located in family room on first floor. Fire Safety Clearance was granted by the Contra Costa County Fire Protection District Authority on 08/12/2025 for a total capacity of 14. Report continued on LIC 809-C Physical Inspection : The LPAs toured on-limit areas accessible to children. LPAs observed toys, play equipment and materials for children. Licensee stated outdoor play area will be remodeled and the play structure will be removed and replaced. Licensee will submit photos and videos to LPA Canela when completed. Licensee stated there are no weapons or firearms in the home. There is a fully charged 3A40BC fire extinguisher and Pull-down Fire Alarm. Combination smoke and carbon monoxide detectors were observed operational. Disaster Drill was conducted on 05/05/2025. LPAs did not observed any bodies of water on the premises. Licensee understands that when outside of facility, 100% supervision of children in care is required. Facility does provide transportation for children; Licensee understands children cannot be left unattended in parked vehicles. Licensee does not provide overnight care. There is a working telephone available. Licensee understands smoking is prohibited on the premises. Record Review : The facility roster was reviewed and a copy was obtained. Required postings were observed near the entrance. Licensee/Applicant has current Infant First Aid/CPR/AED which expires 2/2026. Licensee/Applicant completed required Mandated Reporter Training and it expires in 1/2026. Licensee/Applicant completed 8 hour EMSA Preventative Health & Safety training. Applicant completed 8 hours EMSA Training for Nutrition and Lead Poisoning. Licensee/Applicant has required immunizations. Applicant and adults living in the home have Criminal Record Clearance and required Tuberculosis (TB) immunization. LPA reminded licensee day care needs to be operated within the limitations and capacity of a Large family Child Care Home with regard to ratios and Licensee is required to be present in the day care for 80% of the operation hours. Applicant understands that children's personal rights should not be violated and no corporal punishment. Isolation of sick children, supervision of children, capacity options, transportation of children, requirements for reporting suspected child abuse, unusual incidents/injuries and requirements for assistant/substitute were also discussed. Fire drills must be conducted once every six months and documented. The facility currently does not have any children in care needing Incidental Medical Services \342\200\223 IMS. Mandated Reporter Training : Any person who works in a child care facility shall complete and renew the training every 2 years. Criminal Record Clearance or Exemption: Licensee/Applicant understands all adults 18 and over residing, living in, or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum per day, per individual, for a maximum of 5 days will be assessed for violation of regulation for uncleared individuals residing or being present in the facility. Application is complete. Applicant is approved for increase in capacity for a Large Family Child Care Home effective as of today 09/04/2025.
