Routine inspection (3-year)·October 17, 2025

On 10/17/2025 at approximately 1:30PM, Licensing Program Analyst (LPA) Jamel Maiwandi met with Licensee Zohreh Ashrafi Ghorban for an unannounced required 3 year annual inspection. Present during today\342\200\231s inspection were the licensee, her spouse who is also her assistant, and 6 children. The licensee and her spouse are both fingerprint cleared. Hours of operation are from 7:00am-5:00pm Monday to Friday. The home is a one-story house and consists of the living room, dining room, 4 bedrooms, 3 restrooms, laundry room, backyard and garage. On-Limit Areas : Living room, dining room, kitchen, restroom 1, bedroom 1 and left side of backyard. Off-Limit Areas : Master bedroom, restroom 3, restroom 2, bedroom 2, bedroom 3, laundry room, garage and right side of backyard. LPA inspected the indoor area and observed all off-limit areas to have been made inaccessible by closed and/or locked doors and visual supervision . Areas accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for safety and comfort. There are ample number of safe toys, play equipment, and materials observed for children. There is a working telephone in the home. LPA verified all poisons, cleaning solutions, medications, and other items that pose a danger to children are locked and inaccessible to children. The licensee was reminded that all cleaning compounds, medications, and poisons must be in a locked cabinet/drawer or placed out of reach of children. Continues on 809-C Page 2 Knives and kitchen utensils were kept in a locked storage cabinet. The home is equipped with a fully charged 2A10BC fire extinguisher and working carbon monoxide detector. There is a fully stocked first aid kit located in the home. Licensee stated there are no firearms on the premises. LPA inspected the outdoor area (backyard) and found it to be fully fenced. LPA observed a shed that is locked and inaccessible to children. There are no pools, spas, hot tubs, fishponds, or similar bodies of water located in the home. LPA observed the right side of the backyard to be separated by a locked gate and off-limits to children. The licensee conducts and documents fire and disaster drills twice a year with the last one conducted on 10/9/2025. The Licensee\342\200\231s and spouse\342\200\231s CPR and First Aid certificates are current and expire in 08/2026. The Licensee and spouse both have current mandated reporter training which expires on 1/23/2026. The Licensee was reminded of the responsibility as a mandated reporter. LPA reviewed 2 staff files and 6 children\342\200\231s files at 2:30pm and obtained a current facility roster. The licensee is in compliance with the immunization law. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPAs also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Continues on 809-C Page 3 Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Zohreh Ghorban confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
