What the inspector wrote
On 12/8/23 Licensing Program Analysts (LPAs) Brindha Govindasamy and Monica Mathur conducted an announced Pre-licensing inspection and met with applicant Zahra Hedayat Khouzani and her spouse. Residing in the home are the Applicant, her spouse and 2 minor children (age 9,6 years old). The Applicant plans to operate Monday through Friday from 8am to 5pm. This home is a single story house with a detached Accessory Dwelling Unit ADU in the backyard with separate access pathway from the front of the home. Applicant is reminded that when her children are present in the home during day care hours, they will count in day care ratio requirements.
Applicant states they will be using the ADU portion of the home as Play/Activity area for day care. Children will use the Main House Play Room for eating and nap time. She was reminded that ADU area can only be used for activity space, and napping/eating is not allowed in that unit. Children will be dropped off at the Main House front door, they will be walking to the ADU through the front yard, past the side gate, down pathway to access the ADU.

The home was observed to be neat and clean with heating and ventilation for safety and comfort of the children. There are age-appropriate toys which are observed to be safe, clean and in good repair. The Applicant plans on providing lunch and snacks to the children. There is a small water fountain in the off limit yard area outside the Main House Play Room which is not in use. LPA did not observe any hazardous materials or toxins accessible to children during today\342\200\231s inspection. There is a screened/barricaded fireplace in the main house living room. Per Applicant there are no firearms in the home. Off limits areas are made inaccessible by closed and/or locked doors, gates, visual supervision, strap latch for drawers. There is a chicken coop with live chicken in the off limit backyard. Applicant was reminded that children must not be in the off limit backyard areas.
The home do not have a fully charged 2A10BC fire extinguishers. The main house and ADU have working dual smoke and carbon monoxide detector and a working telephone. The applicant\342\200\231s CPR certificate expires on 8/19/25 and the Mandated Reporter Certificate expires on 9/4/25.
Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Applicant provided proof of control of property.

LPA reviewed with Applicant LIC 311D, Forms/Records to Keep in Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.
LPA discussed the safe sleep regulations with Applicant and discussed the Child Care Licensing Safe Sleep webpage at:

as an additional resource. LPA also informed Zahra of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 12/8/23, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Applicant agreed to make the following corrections before license can be granted:

3. Barricade 2 air conditioning units in the side yard pathway leading to ADU
4. Fix a broken fence slat on the on limit area of ADU yard
5. Install child gate to barricade access to hallway bedroom in ADU
6. Fire Extinguisher 2A10BC or up
7. Postings on wall near front door.
The family home is recommended for Small Family Childcare Home license subject to corrections. Exit interview conducted and report was reviewed with the Applicant Zahra Hedayat Khouzani.
