Inspector’s narrative

What the inspector wrote

On 9/3/25 at 12pm, Licensing Program Analysts (LPAs) Catherine Fernandes and Indira Loza arrived unannounced and met with the Licensee's assistant Marta Perez. Present in care were four infants, two preschoolers and one school age child. Licensee Glady Hernandez arrived 20 minutes later with her two children, one school age and the one preschooler. Upon arrival the LPAs observed three infants watching tv, one of the three infants were strapped into a car seat and the other two infants were in playpens. LPA Loza explained to the licensee's assistant that the three infants can not be restrained while in care and was asked to remove the infants from the car seat and the playpen. LPAs also informed the Licensee that her home was considered to be out of ratio upon our arrival and that if there is only one caregiver in the home then the home needs to follow the requirements for a small family child care license. Licensee stated she left to pick up her children from school. LPAs Fernandes and Loza informed licensee Hernandez that this report dated 9/3/25 documents two Type A citations which shall be posted for 30 consecutive days as there is an immediate risks to the health, safety, or personal rights of children in care. Also, LPAs informed the licensee to provide a copy of this licensing report dated 9/3/25 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted with Hernandez. Report, Appeal Rights, LIC9224, and Notice of site visit provided. See 809D for deficiencies.
