Inspector’s narrative

What the inspector wrote

On 06/26/2025 at 11:30 AM, Licensing Program Analyst (LPA) Christina Watts conducted a Case Management Inspection at Baby Yale Academy at BP. LPA met with Teacher/Director, Dionna Knight and explained the purpose of this visit. During today's visit, there were no infants in care with 2 staff in 2 classrooms. Director stated there are 6 infants enrolled. All staff caring and supervising children have Criminal Record Clearance. LPA was conducting a a visit regarding an incident that occurred in the facility. On 06/13/2025, around 1:00 PM, infant children were waking up from a nap. S1 stated they changed C1's diaper and placed C1 on the floor. S1 stated as they were walking to the trash can, when they heard C1 crying. The Director stated they heard a boom in the classroom and went to assist. S1 stated C1 had fell on the bottle. S1 stated they pulled C1's hair from their face and seen a cut on C1's left eyebrow. S1 stated C1 was bleeding but their was not a lot of blood. S1 stated they went to console C1. The Director stated they asked S1 what happened and S1 explained what happened. Director stated they contacted C1's parent to inform them of the incident. Director stated C1's parent came to pick up C1 about 30-40 minutes later. Director stated they informed C1's parent of what happened in person and gave C1's parent an incident report. Director stated they did not inform licensing because they informed C1's parent and have a signed incident report. Per California code of regulations, Title 22, all facility are to report any incident where a child injure themselves and they have an open wound and/or bleeding. The facility is in violations of California code of Regulations, Title 22. LPA Christina Watts informed Director, Dionna Knight that this report dated 06/26/2025 documents a Type B citation. Type B citation(s) are a potential risk(s) to the health, safety, or personal rights of children in care. *CON'T ON PAGE 2* *PAGE 2* *SEE LIC 809-D FOR DEFICIENCIES* Exit interview conducted and report was reviewed with Director, Dionna Knight. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
