What the inspector wrote

On 07/31/2025 at 12:10 PM, Licensing Program Analyst (LPA) Christina Watts conducted an Unannounced Subsequent Complaint Investigation at Baby Yale Academy at BP. LPA met with Teacher, Monica Perez to deliver findings for the above allegation. During today's inspection, there were 1 infant child in care with 1 staff in 1 classrooms. Teacher Monica stated there are 6 children enrolled. Complainant alleges that Staff did not provide adequate supervision resulting in child sustaining injury while in care. During the course of the investigation, LPA inspected the facility, reviewed records and conducted interviews. It was determined that staff did not provide adequate supervision which resulted in C1 sustaining an injury. Based on the interviews and information obtained throughout the investigation, the preponderance of evidence standard has been met. Therefore, the allegation is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 is being cited on 9099-D page. Exit interview was conducted with teacher, Monica Perez. Appeal rights were provided. A NOTICE OF SITE VISIT WAS ISSUED AND MUST BE POSTED FOR 30 CONSECTIVE DAYS.
