On September 8, 2025 at 12:30pm, Licensing Program Analyst (LPA) Julia Placencia arrived at the facility unannounced for an Annual/Random Inspection. LPA met with director Rachel Liles. There were ten infants and an additional three staff members present during he inspection. The facility was toured for a health and safety inspection. The hours of operation are Monday through Friday, 7:30am to 5:30pm. Physical Plant: Furniture and napping equipment observed during inspection were age appropriate and in good condition. Each crib is occupied by only one infant at a time, and is free from all loose objects, including blankets and pillows. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children were inaccessible during the inspection. The sinks were clean and in operable condition. The floors were free of tripping hazards. Kitchen/food preparation and storage areas were observed to be clean, and free of evidence of rodents, and food is protected against contamination. All storage containers for solid waste have tight-fitting covers that are in good repair. Drinking water is available both indoors and outdoors. Infants bottles and food containers were labeled with current names/dates. Required licensing documents were posted. Fire extinguishers, carbon monoxide and smoke detectors meet State Fire Marshall standards. The last fire/disaster drill was conducted on 4/14/25 and meets the six month requirement. The center is in compliance with the sign in and out procedure. Outdoor Activity Space: There are no pools or similar bodies of water at this facility. Outdoor activity space and playground equipment were observed to be safe, free of hazards and had appropriate cushioning material to absorb falls. There is a shaded area provided for the children. ***Continued on LIC809-C... Ratio/Capacity: The facility is operating within its licensed capacity. The facility is within ratio today with one teacher supervising no more than four Infants. LPA did not observe any child left without visual supervision during the inspection. Records Review: A same of children and staff files were reviewed. Infants up to 12 months have Individual Infant Sleeping Plans, and the facility maintains a sleep log for infants up to 24 months. Teacher present today meet the qualification requirements. LPA verified both opening and closing staff have current CPR/First aid training. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. ***Continued on LIC809-C... LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. The following deficiencies were observed during today\342\200\231s inspection: Infant's Needs and Services Plans are not consistently updated at least quarterly. Staff S4 does not have proof of measles and pertussis. Staff S1 and S2 have expired mandated reporter training (S1-8/4/25, S2-10/4/24). See LIC809D for deficiencies cited during today's inspection. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected. *** Please see LIC 9102 for Advisory Notes.
