Inspector’s narrative

What the inspector wrote

On May 6, 2026 at 8:30am, Licensing Program Analyst (LPA) Julia Placencia arrived unannounced for an Annual/Random Inspection. LPA met with teacher Khyati Parikh as licensee Anuradha Sala was on jury duty. There were 21 children and an additional 3 staff members present during the inspection. The facility was toured inside and outside for a health and safety inspection. Hours of operation are Monday through Friday, 8:00am to 6:00pm. Physical Plant: Disinfectants, cleaning solutions, poisons and other items that are dangerous to children were inaccessible during the visit. The toilets and sinks were safe, sanitary and in operable condition. Furniture and equipment are in good condition, and floors are free of tripping hazards. The kitchen/food preparation and storage areas appear to be clean and free of evidence of rodents, and food is protected against contamination. Storage containers for solid waste have tight-fitting covers that are in good repair. Drinking water is available both indoors and outdoors. The facility provides lunch and snacks, and menus are posted for parents to view. Medications are inaccessible to children. Outdoor Activity Space: Outdoor activity space and playground equipment was observed to be safe and free of hazards. Climbing equipment is properly anchored to the ground with adequate and appropriate cushioning material to absorb falls. There is a shaded area provided for the children. There are no pools or similar bodies of water at this facility. Ratio/Capacity: The facility is operating within its licensed capacity. The facility is within ratio today with one teacher supervising no more than 12 children. LPA did not observe any child left without visual supervision or unattended during the inspection. ***Continued on LIC809-C... Records: A sample of children files were reviewed and contain required licensing documents. Files of staff present today were also reviewed. Teachers meet the qualification requirements. Fire/Disaster Drills are conducted at least every six months, and a review of the log shows the last drill was conducted on 2/5/26. LPA verified both opening and closing staff have current CPR/First aid training. The center is in compliance with the sign in and out procedure. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
