An unannounced case management inspection was conducted today at 10:45 am by Licensing Program Analyst (LPA) Kiriko Lynch. LPA met with facility representative Tara Salvaressa in response to an Unusual Incident Report received by the Department on 03/23/26 which was self-reported by the facility representative. The incident was regarding a child (C1) that was left unattended on the playground. The facility representative was interviewed on 04/06/26 at 1:30 pm and stated that on 03/20/26 at 11:15 am, C1 was left outside on the playground by staff during a transition from outside to inside. Facility representative stated C1 was left outside for a period of less than a minute in a fenced area, was uninjured, and parent was notified of the incident and C1 remained in care. She stated they conducted a refresher training for all staff in regard to the supervision to be provided during transition periods, including conducting daily counts of children outside and inside and updating the white board and daily roster as needed. Five witnesses (W1-W5), including staff from facility and adjacent elementary school were interviewed on 04/28/26 and 06/04/26, and witness disclosures generally revealed C1 was left outside during a transition period for approximately less than a minute, was not injured or upset, and remained in care at the facility. During today’s inspection, the facility was toured. LPA observed the playground areas which are fenced completely, and also appropriate care and supervision was being provided to children by staff at the facility. Based on interviews, the following deficiency is being cited on the LIC809-D. Exit interview conducted and report was reviewed with the facility representative Tara Salvaressa. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days.
