What the inspector wrote

On May 3rd, 2023, Licensing Program Analyst (LPA) Soleil Marx, met with Director, Robb Staples, to open and close a complaint. Upon arrival, LPA observed 86 preschool students being supervised by nine staff. It was alleged that the Licensee does not ensure outdoor play areas are free from hazards. During the investigation, LPA Marx inspected the facility, reviewed injury reports, and interviewed Director. LPA Marx observed holes/ruts and uneven ground throughout the grass and dirt in the outdoor activity space, which is also unlevel with the cement. Based on interview, observations, and record review, the preponderance of evidence standard has been met, therefore the above allegation weas found to be SUBSTANTIATED. Director submitted a work order on 04/26/23 to level and resurface the outdoor activity space, which is in process to be completed 05/26/23 and that area of the outdoor space is currently not being utilized until the ground is leveled and free of hazards. Title 22 Deficiencies are being cited on 9099-D. Appeal rights were provided and exit interview conducted. The Notice of Site Visit must be posted for 30 days.
