What the inspector wrote

Based on LPA observation, interview, and record review, the preponderance of evidence standard has been met, therefore the above allegation is SUBSTANTIATED. One type A Title 22 deficiency was cited on 9099-D. The Facility Representative was informed that this report dated 07/18/2024 documents one Type A citations which shall be posted for 30 consecutive days. The director shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report reviewed with Facility Representative. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    The licensee shall provide care and supervision as necessary to meet the children's needs.No child shall be left without the supervision of a teacher at any time..Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interview, the licensee did not ensure compliance of the section cited above by not visually supervising children at all times, which poses an immediate Health,Safety, or Personal Rights risk to persons in care.
