A.M., Licensing Program Analysts (LPAs) Arianna Manabat and Karyn Guerra met with Licensee Jennifer Tommasini, for the purpose of a 1 year- required inspection. During today’s visit, LPAs observed 23 infant children supervised by 7 staff. Hours of operation are from 6:30am to 6:00 p.m., Monday through Friday.
LPAs conducted a health and safety inspection of the classroom, restrooms, and food service areas. LPAs observed that the facility had the required postings: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, car seat laws, menus, waivers, and daily schedule. All cleaning compounds and hazardous items are appropriately stored and inaccessible to children in care. LPAs informed the facility that they need to post waivers.
LPAs observed a changing table with raised sides within arm's reach of a sink. Bins for solid waste have tight fitting lids. The floors appeared clean throughout the facility. Facility provides formula and breastmilk for infant children. LPAs observed bottles labeled with children's names. Other food in the facility was properly stored to prevent contamination. Drinking water is provided to children via labeled water bottles in caddies. LPAs observed sign in and sign out logs are done virtually. There are no firearms, poisons, or bodies of water on the premises.
LPAs conducted a health and safety inspection of the outdoor space and playground equipment. The surfaces were free of loose or sharp parts. LPAs observed wood chips to provide cushioning. Outdoor shade was provided by trees. Water for drinking is accessible indoors and outdoors.
At 12:55 p.m., LPAs observed three infants (C1, C2, C3) who are unable to climb out of a crib sleeping on cots in the One's classroom. This poses an immediate threat to the health and safety of children in care.

LPAs reviewed staff and children's records. Each child's file contained an admission agreement, emergency card, health history, immunizations, and acknowledgement of receipt of personal rights, consent for emergency medical treatment, and parent’s rights. LPAs observed valid First aid and CPR certification for at least one staff on the facility premises. LPAs observed current staff immunization, employee rights, and documentation of the educational background, training, and/or experience. Licensee was informed that Mandated reporter trainings need to be conducted every two years.
Staff was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPAs discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee.

Director's signature on this report acknowledges receipt of these rights. This report was reviewed with the Director. An exit interview was conducted with Director, Diana Wise. A Notice of Site Visit was provided and shall remain posted for a period of 30 days.Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
