What the inspector wrote

On May 14, 2025 Licensing Program Analyst (LPA) Soleil Marx, met with Director, Amy Lindstrom, for the purpose of conducting an unannounced case management inspection to follow up on a self reported incident. The purpose of today's inspection was explained. During today's inspection, LPA observed a census of 15 preschool age children being supervised by three staff. The facility reported to Sacramento Regional Office that on May 8, 2025, a child tripped and fell while running on wood chips in the outdoor activity space which resulted in the child sustaining a broken ankle/leg. The parent was notified immediately following the incident. LPA verified the facility was in compliance with CCR 101212, reporting requirements, as the incident was reported the following day via telephone to LPA Marx and an unusual incident report was submitted. During today's inspection, LPA made observations of facility environment, reviewed records relevant to the incident, and interviewed staff that observed the incident. LPA verified that the outdoor activity space where the incident occurred was in compliance with 101238.2 Outdoor Activity Space regulations. LPA observed playground equipment was arranged so there are no conflicting hazards from activities, there was a sufficient amount of cushioning material (wood chips) to absorb a fall, and surfaces were in safe condition and free of hazards. No Title 22 deficiencies are being cited at this time. Exit interview was conducted and report reviewed with Director. A Notice of Site was provided and must remain posted for 30 days.
