What the inspector wrote

8/7/25 approximately 9:00am Licensing Program Analyst (LPA) Andrea Cortez met with Licensee Rhonda Walker for an unannounced case management inspection to follow up on an Unusual Incident Report submitted to the Duty Officer of The Day (DO) on 7/29/25. LPA observed 16 children supervised by 5 staff playing on the playground. During today\342\200\231s inspection, LPA conducted observation of the classroom where the incident occurred. On 7/29 Director self-reported to the DO a child fell from a chair and paramedic where called. LPA interviewed Licensee and Aide regarding the incident. At this time further investigation is required. This report was reviewed with Licensee Rhonda Walker. A notice of site visit was provided to be posted for 30 days.
