What the inspector wrote

Licensing Program Analyst (LPA) Andrea Cortez met with Licensee Rhonda Walker for an Unannounced Case Management-D eficiency to deliver findings on an Unusual Incident Report submitted to the Duty Officer of The Day (DO) on 7/29/25. On 7/29 the director reported a child was sitting at a table playing when child fell out of the chair and hit their head. Director stated one of the teachers did not think the child was responding well because her eyes were rolling back. The paramedics were call and the child was transported to a nearby hospital by parents. On 8/7 LPA inspected the classroom furniture and found the chair to be inappropriate. LPA examined the child while sitting in the chair and found child\342\200\231s feet where not touching the ground. LPA explained the A Deficiency violation: Personal Rights 101223 (a)(2) (a)The licensee shall ensure that each child is accorded with the following personal rights: (2) To be accorded safe, healthful and comfortable accommodation, furnishings and equipment to meet his/her needs. An exit interview was conducted with Rhonda Walker , a copy of this report, with the 809-D was given to the Director. Notice of the site visit was given and must remain posted for 30 days. See 809-D.
