What the inspector wrote

On February 11th, 2026, Licensing Program Analyst (LPA) Ye Vang & Amanda Sutter met with Director Ocean for the purpose of an unannounced Case Management inspection. LPA observed 18 children supervised by 3 staff. On January 26th, 2026, a UIR was submitted, in regards to a injury that accrued. Child 1(C1) was climbing up the ladder if the play structure and slipped, falling onto the ground. Staff 1(S1) stated she was standing in the garden area when she observed C1 fall. Staff 2(S2) stated that she was standing by a pole on the other side of the playground and stated she did not observed the fall. S2 went over to C1 to check on the child and took the child over to S1, who immediately contacted C1's guardian. As C1 and S1 was waiting S1 took C1 into the building to check for injuries and also applied ice pack. After medical examination, facility was notified that C1 broke their tibia. No deficiencies were cited in today’s visit. Exit interview was conducted with Director Ocean Greene and notice of site visit was posted.
