Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Christopher Bello arrived at the facility at approximately 10:40am and met with Director, Mireya Torrez. During an inspection LPA observed two infants sleeping in the nap area and two other infants whom were awake with one teacher present. This is considered as an immediate risk to the children in care. Title 22 Deficiencies have been cited on the attached LIC 809D . Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 809D in each child's files. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Mireya Torrez.

    There shall be a ratio of one teacher for every four infants in attendance. This requirement has not been met by evidence: LPA observed two assistants with infants without a teacher present. This is considered as an immediate risk to the children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION FOR INFANTS

    Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement has not been met by evidence: LPA observed one staff with two sleeping infants and two awake infants. This is considered as an immediate risk to the children in care.
